At Gastroenterology Associates, our medical staff is dedicated to providing you with high quality care and making you comfortable throughout your visit. The information below will prepare you for your visit and make it go more smoothly.
For Your First Office Appointment:
Bring a photo I.D. and your most recent insurance cards, including prescription cards if applicable.
Complete all intake paperwork prior to your appointment. If you cannot complete in advance, please arrive 15 minutes early.
Bring your medications or a list of your current medications with dosage and frequency.
- Bringing test results, lab work, X-rays, etc., that pertain to the reason for your visit is encouraged. The doctor will ask you questions about previous illnesses and medications. Your answers may help to successfully treat your present medical condition.

During Your Office Visit:
A physical examination, especially directed to the areas pertaining to the gastrointestinal tract, will be performed. The doctor will then discuss further diagnostic tests, such as laboratory studies, X-rays and endoscopic exams that may help diagnose your condition.
Insurance Guidelines
Gastroenterology Associates is a participating provider with Medicare and several managed care insurance plans, and we follow the rules set forth by those plans. You are responsible for all deductibles and co-payments for your plan at the time of service. In preparation for your appointment you should know the following:
- If a referral is required. We do accept self referred patients.
- If our facility or provider are in-network.
- Your annual deductible or co-payment.
